This kit is the Madoka Yuki Swim Style from Kotobukiya's Sōsai Shōjo Teien series. As the name suggests, it's Madoka in a bikini. Painted parts have been applied to a number of pieces, including the polka-dot bikini and sandals.

The overall parts lineup is the same as the winter/summer uniform versions, but the big difference here is obviously the swimsuit. You still get the same 3 hairstyles as before, though this time a flower accessory has been added to the front hair. On top of that, 3 types of bare feet are included in addition to the sandaled feet, and straight arm parts are also swappable so the articulation joints stay hidden when the arms are extended. A bent-forward waist part is included as well, making it easy to pull off a natural pose with knees raised while sitting.
